• Students enrolling in SMS must present the following items:

    1. Birth Record to prove age of student,

    2. student's Social Security Card,

    3. up-to-date Alabama Immunization Certificate (expired shots cannot be accepted),

    4. a copy of the Parent's/Legal Guardian's Driver's License,

    5. a Court Decree declaring guardianship to the person registering the student if the student does not live with parents or if parents are divorced,

    6. two proofs of residency, showing that the parent or legal guardian lives in Lee County, which may include:

    *Apartment/Home Lease Agreement

    *Home Purchase Agreement (House Notes)

    *Utility Bill (water, power, phone, cable)

    *Property Tax Record

    **All information must show the Lee Road address and the Parent/Guardian name,

    7. a Notarized Resident Affidavit must be presented along with two proofs of residency if the student resides in someone else's home other than the parent or legal guardian registering the student,

    8. a Withdrawal Record from the student's previous school, and

    9. all of the above for any student who is re-entering after interrupted attendance.
