This year, Health Heroes will be coming around to all Lee County Schools on Tuesday, October 22, 2024 to administer free Flu vaccines to students. If you would like for your child to receive this vaccine, please print out the form below, fill it out completely and return to your child's teacher no later than Friday, October 18th. If you are unable to print this document, you may contact the school nurse at your child's school, and they can send a form home with your student. Students who are covered under BCBS PEEHIP insurance are not allowed to participate in this program, but they may receive the vaccination when the PEEHIP clinic comes to their school.
